Expense claim

One entry per receipt. A valid receipt shows the vendor, date, each item and the total. A card statement alone isn't enough. For an emailed receipt, take a screenshot and add that.

I certify that these expenses were incurred for Harvard Club of Dallas activities, that no one else has reimbursed me for them, and that the receipts and statements attached are accurate. I will repay any amount paid to me in error.
